Identity and Statement of Educational Purpose 2022-23

The Identity and Statement of Education Purpose can be provided to the Financial Aid office in one of two ways.

• Completing the form in the financial aid office. Be sure to bring with you an unexpired valid government-issued photo identification (ID), such as, but not limited to, a driver’s license, other state-issued ID, or passport.

or

• Mailing, to the Financial Aid office, the signed original Identity and Statement of Educational Purpose that has also been notarized. Make sure to include a copy of your unexpired valid government-issued photo identification (ID).
